Form 1: Checklist for Adult Sponsor / Safety Assessment Form - REQUIRED for ALL PROJECTS

Form 1A: Student Checklist / Research Plan - REQUIRED for ALL PROJECTS

Form 1B: Approval Form - REQUIRED for ALL PROJECTS + All students need parent/guardian approval

Form 1C: Regulated Research Institutional / Industrial Setting Form - REQUIRED ONLY if you are performing the experiment in a formalized institution

Form 2: Qualified Scientist Form - REQUIRED for experiments involving Human Participants, Potentially Hazardous Biological Agents, Vertebrates, DEA-Controlled Substances, and having a Scientist oversee your project.

Form 3: Risk Assessment Form - REQUIRED for ALL Projects

Form 4: Human Participants and Informed Consent Form - ONLY FOR HUMAN PROJECTS that require human participation

Form 5: Vertebrate Animal Form (5A and 5B) - ONLY for VERTEBRATE ANIMALS

Form 6A: Potentially Hazardous Biological Agents Form - IF YOU USE ANYTHING that you are not sure of, fill out this form to be on the safe side

Form 6B: Human and Vertebrate Animal Tissue Form - ONLY IF STUDYING TISSUE

Form 7: Continuation Projects Form - ONLY if you are continuing a research you've done in previous years
