Yes, Maxie will ask for your Card Number and Birthdate to verify instead of using Maxicare SSO Account. We still encourage you to sign-up for a more convenient way of submitting your request to Maxie.

For affiliated clinics/hospitals, Maxie will require you to submit a Doctor’s Request from a Maxicare-affiliated doctor. 

But if you go to a Maxicare Primary Care Clinic or Corporate Clinic, you don’t need to to present a Doctor’s Request or Letter of Authorization.

• LOA with an availment beyond 3 days - Day before the availment

• Homecare booking confirmation - Within 24 hours
