Marywood History and Physical Form
QuantiFERON Gold lab test
Immunization history consisting of: Completed Hepatitis B series, Polio series, Completed DTP series, Tdap within 10 years, and annual Influenza vaccine (due every November). While the COVID-19 vaccine is not a requirement of Marywood University, PA students cannot be placed in community activities or clinical rotations without being fully vaccinated. If students are unable to fulfill required portions of the program, including clinical rotations and other activities, they will not be able to graduate.
Titers: The titers that are required by our clinical sites include Hepatitis B Surface Antibody, Measles, Mumps, Rubella, and Varicella IgG titers. Results must be submitted to the Student Health Portal.

Student health insurance is mandatory for all full-time students. The policy’s annual cost appears as a separate item on the semester bill. Students who have comparable health insurance coverage may choose to waive enrollment in this policy. To complete the waiver process, students must demonstrate proof of their insurance by providing current plan information (on the insurance ID card). This process is completed prior to the fall semester. Once proof of insurance is verified, the charge will be removed. Any student who does not complete the waiver process by the annual fall deadline will be covered through the university’s policy. The charge will remain on the student’s account and must be paid.
