The applicant must:
1. be enrolled for this Academic Year 2021-2022
2. complete the Online School ID Application Google Form using the link https://tinyurl.com/ytb2kyav
3. attach to the form a half-body photo taken recently in full-face view, directly facing the camera [front view] against a green, blue, or contrasting-colored backdrop or background
4. observe the Dress Code and Haircut for Male & Female
5. attach to the form your scanned signature on clean white paper using a black 0.5-tipped sign pen
6. wait for an email from the Student Affairs and Services [SAS] Office with information on when the ID will be ready to pick up.
7. The IDs will be distributed in batches every Friday from 8:30 AM to 12:00 NN and 1:00 PM to 4:30 PM at the Main Campus, School Canteen area.
8. In case of loss, the applicant must secure the following:
- Affidavit of Loss
- Receipt of ID replacement application
- Other requirements as outlined above
