In accordance with best practice, as recommended by state statute, the WI Department of Public Instruction and MDIH School Board Policy 420, the district verifies residency information annually. This is the last step in the registration process.
*NOTE: If you are new to the district, enrolling for the first time this summer, you have already completed this step.
Please be prepared to upload one document from EACH of the following lists as you complete the online registration process.
List 1 (including resident address)
Current property tax statement (available online through your village/city hall
Closing statement for a newly-purchased (within the previous 12 months) home
Current, signed residential lease (including landlord's name, address, and phone number; if your lease is month-to-month, you must provide August 2020 lease
List 2 (including resident address)
Auto or health insurance statement
Driver's license renewal notice
License plate renewal notice
Food Share/Quest benefits notice
Medicaid/Badgercare benefit statement
W-2, SSI or other county, state, or federal benefit statement
