Physicals & Concussion Info

All MJHS student athletes must have a current physical on file at the school as well as have a parent sign off on the concussion information documents prior to participating or trying out for any sport at MJHS.

All athletes must have a current physical on file. Athletes may use this form to complete a sports physical. This does not meet the requirements for a school physical.
