Application Form

Please contact Haley Brown for a copy of the portfolio application form. When you submit the form, please include the payment of the portfolio fee ($50/credit hour, ex: 3-credit course = $150). You can either write a check (made out to UMA), or can pay via debit/credit card over the phone.

Haley Brown will update the form on the site as the review process progresses (payment, review, final decision, signatures and posting of credit, if applicable).
