Application Form

Please contact Haley Brown for a copy of the portfolio application form. When you submit the form, please include the payment of the portfolio fee ($50/credit hour, ex: 3-credit course = $150). You can pay via the following options:

• Debit/Credit Card: Please call UMA Enrollment Services (207-621-3185) to pay with a card over the phone.

• Check/Money Order: Please make checks out to UMA and mail them to the address below:
