The PLS Questionnaire is an assessment to identify what skills a child needs to learn from the list of 13 preschool life skills. This allows the instructor to focus on the skills that the child does not currently have in their repertoire. Once the PLS curriculum is started, it can be used to track a child's progress.

The original PLS Questionnaire may be printed or downloaded as a fillable form.  A modified PLS Questionnaire is being piloted and may be used with the understanding that the published research utilized the original form.
