New students enrolling at Madison Park Middle School will need to provide the following documents, along with a completed Enrollment Packet. (Enrollment packets can be picked up at the District Office, Park office, or download from the District Website under the Enroll tab.)

Immunization Record
Proof of Address (Parent/Guardian name must be on proof of address)
Custody Papers, if applicable
Withdrawal Slip from previous school
SPEd (IEP), if applicable
