Work Permits

FYI: You will need your parent or legal guardian for this step!

1. Click here. A new window will open to the DWD "Steps to Create a Work Permit" website.

2. IMPORTANT: Your parent or legal guardian will need to register for an account before they can apply for a work permit.

   1. 1. • If you already have a DWD account you can log in using the "Create a Work Permit" button

3. Once you have created or logged into your DWD account you should be on a page that says "Create a Work permit." (If you are not, click on "Create a work permit")

4. You will then completed the needed information on each screen (Please see the Steps to Create a Work Permit guide for details)

         • Employer

           1. Name: MSCR

           2. Zip code: 53715

           3. Click "Search"

           4. Select " MSCR 328 E LAKESIDE STREET MADISON, WI 53715" (Click here to see a screen)

           5. Click "Use selected Employer"

***DO NOT Create a New Employer***

4. 1. 1. • Work

           1. Nature of Business: Recreation

           2. Type of Work: try keys words that describe the job you have been offered with MSCR. Examples: Child Care, Camp Counselor, Art/Dance Instructor, Assistant Instructor, Referee (Public Schools).

         • School

         • Review (review to make sure all answers are correct)

5. You will then proceed to payment and pay.

-MSCR reimburses the $10 permit fee after completion of all paperwork

6. Download a copy of your work permit (Directions)

7. Submit your work permit with your other paperwork (email to your Program Assistant)
