Have your parent / guardian complete the parent online permissions here by logging in using their infinite campus username and password
Turn in proof of a recent physical to the Athletic Office at East High School
-WIAA Physical Form-click to download (usually due before freshman & junior years)

WHAT DO I NEED TO DO TO PLAY SPORTS AT MADISON EAST?
A prospective Student-Athlete must complete the following before being able to participate in practice or competitions –
** The student and the family must each complete their sections of Online Athletic Registration Form (click to visit)
(using their respective Infinite Campus credentials-click for more information)
please note – there is one section for the Student and a different, separate section for the family to complete.
** Have proof of a recent physical*, valid for the current school year, on file with the Athletic Office –
WIAA Physical Form-click to download (usually due before freshman & junior years)
WIAA Alternate Year Form-click to download (usually due before sophomore & senior years)
A Physical performed after April 1 is good during the school year it was performed & the following two school years.
Ex – dated Apr 4, 2023, was good for remainder of 2022-23 school year, and is good for the 2023-24 and 2024-25 school years
A Physical performed before April 1 is good during the school year it was performed & the following one school year.
Ex – dated March 29, 2023, was good for remainder of 2022-23 school year, and is good for the 2023-24 school year.
