You need your social security number to fill this out online.
If you do not have a social security number, you can complete a paper application (pick up at the Welcome Desk at Madison College or see your school counselor).Application Process:
Find a college or university of interest and then begin your research. Check the admissions page for due dates, eligibility requirements, and supplemental materials to be submitted with the application. You can appeal the requirements based on achievements in other areas. Work with your advisor and your school counselor to determine the best way to communicate to the institution. Ultimately, it is up to the institution to determine your eligibility. Check to see if the institution allows transfer credit. You may need to attend a technical/community college to gain credentials prior to being able to attend your institution of choice. Be sure that the college/university in which you have interest will accept transfer credits from Madison College or another technical/community college.
When taking the ACT students are able to select up to four schools to send their score to for free. To send scores after testing, follow the guide from ACT.
Transcripts may be ordered at Parchment.com by MMSD graduates, parents and guardians of students, and by current students (who are over the age of 18). This electronic service allows you to order transcripts online at any time and track the status of the transcripts that you have requested to be sent.
Choose one of the categories below for more information, or go directly to Parchment. com:
Instructions for current MMSD students (or the parent/guardian of a current MMSD student).
Instructions for graduates of the Madison Metropolitan School District.
Students are eligible for 4 fee waivers (if free/reduced lunch). It works best for students to create the parchment.com accounts first and then get the fee waiver assigned before they attempt to make an order - the students should make sure:
To set up their account with their name exactly as it is listed in Infinite Campus.
To make sure their graduation year is accurate.
To choose the right high school.
To make sure additional identifying personal information (esp. Birth date, etc.) is accurate.
