This is the application form. Once you have completed the three forms from above (ACS Catalina Permission Slip Nov 2025, Catalina LVUSD Permission Slip Nov 2025, and Snorkeling Agreement Nov 2025), please go to this link and complete the Google Form. You will upload the signed forms using this link.
This form is ONLY for students who take ANY medication. If your student takes no medication, you don't need to fill this out. If you have any questions, please email Mrs. Lynch at tlynch@lvusd.org
