6TH GRADE BEACH PERMISSION SLIPS- Beach trip dates for the 2024/2025 school year are October 22 and October 29. Please PRINT, fill in BOTH FORMS and deliver the forms to your PE teacher. We cannot accept electronic forms for this trip. Deadline for permission slips is October 8. We only have space for 30 students per trip so this will be a lottery.




Please wear bathing suit to school, fill in the request for lunch if you want a free bagged lunch, donations are appreciated.
