Step 1: Download Signature Forms and Complete
Two choices for completion/submission:
Print/sign: You will then need to take a photo or scan the documents in order to upload to the Google Forms application submission portal (below).
Complete/sign digitally: You can use Google Drawing for signatures and upload forms from your Google Drive to the Google Forms application submission portal (below).
Give yourself and your recommenders plenty of time to complete and sign. Do not risk having someone say "no" to you because you waited until the last minute.
Ask teachers and adults who know you best and tell them they are signing in support of your scholarship and character.
It's nice if you fill out your own information first before passing it on to the recommenders; this streamlines the process on their part and is generally appreciated!
All teachers must be from GHS unless you transferred from another school this year.
All character recommendations must come from an adult who is not your parent/guardian/relative.
The same person cannot sign both the teacher form and the character form.
Step 2: Complete the Application and Upload All Forms
You must use your LVJUSD student Google account to complete the application form.
All questions are required. You'll be sharing your experience in the following areas: past leadership, jobs, co-curricular activities, awards and achievements, and past volunteer or service experiences. Be prepared to offer details on those. Do not provide activity/ experience information earlier than 9th grade.

Step 3: Drop-off Your Dues
Before the deadline, drop off your $20 dues to Mr. Gresham in Room 101; please place your dues in an envelope with your name on the outside. In the event an application is denied, the dues will be returned to the student. Your application will not be considered complete until you have paid your dues or talked with an NHS advisor; if you are financially unable to submit dues, the club will support you.
Step 4: Wait
Now, you wait! Both the adviser and the Faculty Council members must approve all new members and returning members, so please ha patience with us!

You must use your LVJUSD student Google account to complete the application form.
If you were notified that you are on probation—either due to not completing service hours last year or because your GPA dipped below the 3.8+ cumulative requirement—you'll be given a chance to explain your plan to get back to regular status.
