Rank One

Per LISD policy, ALL athletes including Cheerleaders and Drill Team members must submit online paperwork via Rank One every school year. There are currently five yearly Rank One forms to be submitted; Drug Consent Form, Handbook Acknowledgment Form, Medical History, UIL/LISD Signature Page, and Emergency Card. 

These forms are available only on Rank One. Any athlete who does not have this paperwork submitted, will not be allowed to participate and will be in danger of not being placed in an athletic class period. 


Pre-Participation Physical Information

Lumberton Independent School District, in accordance with UIL rules, requires ALL athletes, including Cheerleaders and Drill Team members, to have a current pre-participation physical and medical history on file before participating in any sport. This includes tryouts, summer workouts, the athletic class period, and game/competition participation. Physicals expire one year from their date; therefore every athlete must TURN IN A NEW PHYSICAL EVERY YEAR. 

Physical forms are available to PRINT AT RANK ONE. 

To ensure all aspects of this examination meet the changing requirements of the State, and to ensure our students have the best possible health care, Lumberton Independent School District no longer offers a mass physical at the school. 

Several local medical offices have offered to be available to offer walk-in services and special pricing for school physicals. The facilities that have agreed to be listed are posted below.

Visiting Team Information

For all games played at Raider stadium:  Visiting teams will be provided a continuous flow water station as well as a cooler filled with ice for injury or other.  Benches will be set up on the visiting team sideline.  

For all sports visiting LISD:  Water coolers and ice will be provided at all events.  If your athletes require taping, we are happy to help them with whatever they need.  However, we do ask that you please send your athletes with their own tape and supplies.

Lumberton ISD employs two, full-time, nationally certified and state licensed Athletic Trainers. These medical professionals are charged with diagnosing injuries/illness, treatment, and rehab. Our athletic training staff aims to keep your child safe and in the game.

LHS Student Athletic Training Program

Our Student Athletic Trainers are a dedicated, hard-working group of young men and women who help to keep our Sports Medicine program running smoothly.  Practices or games, home or away, our students can be found on the fields and in the gyms taping, stretching, bandaging, and helping to keep our athletes at their best.  If you are interested in joining the Student Athletic Training team, please contact Coach Odom or Coach Wilson.
