Medication that must be administered to a student during school hours must be provided by the student’s parent with a prescription from a doctor and a signed permission form from the parent or guardian. All medication must be kept in the nurse’s office in the original bottle with the student’s name and administered by the nurse or another authorized district employee.
Parents should see the campus nurse to drop off medication and sign prescription medication permission forms.
The nurse’s office will not provide or stock any over- the- counter medication. Parents can bring and administer over-the-counter medication to their students as needed.
All students entering seventh grade at Lufkin Middle School must have an up-to-date shot record. This would include the requirements for a Tdap, MCV4, and a Varicella shot for those students who have not had a case of chicken pox or two Varicella shots. In addition, a valid PPD/TB skin test with results is required within the last 12 months (08/01/2016-present) for all new to LISD students and all students entering seventh grade. You will not be able to pick up a schedule in August without proof of completed Immunizations.

A student health information form is included in your online registration packet for your child at the beginning of each school year. It is very important that you complete and return this form immediately. Completing this form gives you an opportunity to list any medical emergency information for the clinic staff to use in order to contact you or a person you designate if consultation is required regarding your child.
Illness at school is evaluated by the health service staff in the clinic area. A child who demonstrates the following symptoms should be sent home from school.
Fever of 100 degrees or above.
Undiagnosed rash over any part of the body or scalp.
Undiagnosed scaly patches on body or scalp.
Vomiting or diarrhea.
Any disease or condition listed on the “Texas Administrative Code” that is listed as exclusion from school.
