• Updated health history information must be completed every fall for every student.  This information is gathered through the  Update Student Information Portal link parents are asked to complete before the start of the new academic year.

• Not completing this updated health information and portal student information may delay or prevent treatment, as parent permission to receive nursing care, treatment, and medications is required.

• Delay of notification of any urgent medical needs may be delayed if the portan student information is not updated with the most current parent contact information.

• The Student Information Portal, including the updated health information, should be completed by the end of the first week of school.

• Students will not be able to attend any school activities including field trips without updated information.
