Transitioning students from school to adult life is an important process for students with IEPs.
Transition services are designed to be a results oriented process that is focused on improving the academic and functional achievement of the student with a disability to help the student’s movement from school to post school activities.
Transition goals and services must be in place no later than when the student reaches age 16, as mandated by the Individuals with Disabilities Act (IDEA 2004). However, in LRSD students receiving instruction in a Community Based Instruction class IEP must include a transition plan no later than the end of 8th grade. This plan also serves as the student success plan.
Post-secondary goals in the transition plan MUST be measurable and based on transition assessments that are provided at least annually!
Your child is invited to every IEP meeting in which transition is discussed.
