Parent Travel Form

Normally, student-athletes should make every effort possible to ride both to and from games with their respective team on the school bus; however, if absolutely necessary to travel home with a parent, please fill out the attached form and turn in to the player's coach to document the situation beforehand.  Thank you for your attention to this matter.
