The "Senior Biography" form will be collected electronically. Seniors, you must sign in to your LPS account to complete and submit your form. The form is due Friday, Sept. 20, 2024, 4 pm. Late bios will not be accepted. No exceptions.

In the Google Form for the Senior Bio, each sport, club, and other activity that has been previously indicated on the paper versions is now a separate question on the form. You must answer each question! If you did not participate in the activity/sport, you must indicate this by selecting the N/A option. If it is an activity/sport that you participated in, simply select the years that you were involved, and move on to the next question. If you participated in an activity/sport that is not indicated on the form, there is space for you to indicate that activity/sport and the years you were involved. If you received any awards or held any leadership positions, you must indicate the years that you received these awards or held these positions.
