Transition Information
Transitioning buildings can often be the most difficult years as students learn new norms and processes. Below is some information on various opportunities and initiatives in place to ease those transition years.
5th Grade to 6th Grade
January/February of 5th Grade Year
Initial Elementary Visits-counselors and principals visit with elementary schools.
Students presented with the enrollment presentation
Flyer sent home with students with a QR code for the enrollment presentation and the google enrollment form. Flyer also put in the elementary newsletter.
Google Enrollment Form needs to be completed by Feb. 18, 2025
Counselors will enroll 5th graders into classes.
March-May of 5th Grade Year
Counselors/Administrators are available for any questions, concerns, course changes, etc that families may have regarding middle school
SVMS Incoming 6th Grade Night-April 23, 2025 6:30-7:30
Counselors/Administrators meet with 5th Grade staff to discuss students with information that needs to be shared with middle level personnel.
IEPs case managers meet to assist with transition to middle school.
June before 6th Grade Year
Summer School Transition Programming
Students can enroll in the LPS Summer School Program and as a part of such, will take a transition class that will help with getting acquainted to the middle level. More details will be available with the Summer School information.
August of 6th Grade Year
August 6, 2025-Registration Day
6th Grade students may come up to the school, get their schedules, pick up their laptop, and take care of any paperwork and fees associated.
August 7, 2025-WEB ORIENTATION DAY
6th Grade Students are invited to participate in a social transition day where they meet other classmates through different large and small group programming. Most of this day is led by 8th Grade WEB Leaders who will serve as mentors to the 6th graders on this day and throughout different activities during the school year.
8th Grade to Freshmen
December/January of 8th Grade Year
December-High School Counselors Present to 8th Grade Students to preview upcoming enrollment process
Incoming Freshman Parent Night @ LNHS
High School Counselors visit SVMS 8th graders to talk high school enrollment and academic/career planning
High School Counselors visit SVMS 8th graders to collect enrollment forms and continue with academic/career planning.
March-May of 8th Grade Year
Counselors/Administrators are available for any questions, concerns, course changes, etc that families may have regarding high school.
Summer School Enrollment for High School is open (more details to come later)
May of 8th Grade Year
Counselors/Administrators meet with High School staff to discuss students with information that needs to be shared with high school level personnel. This will likely include Case Managers as well for students with IEPs.
May TBD - 8th Graders will take a field trip to LNHS during the day to tour the building and hear information from current students about getting involved in the different high school opportunities.
August of 8th Grade Year

August TBD (the day before school starts) LINK ORIENTATION DAY
Freshmen Students are invited to participate in a social transition day where they meet other classmates through different large and small group programming. Most of this day is led by junior/senior LINK CREW Leaders who will serve as mentors to the Freshmen on this day and throughout different activities during the school year.
