College Application Forms

Requesting documents to be sent to colleges can be a confusing process! The information below contains sample forms and screencast tutorials designed to help make sense of it all. Notice every form has a specific color. Color coding allows us to quickly identify what you need when you turn in your form. Please stop by the Office of School Counseling to get original forms on the appropriate colored paper.

Transcript request form - "Pink Form"

The "Pink Form" is used to request documents to be sent to colleges throughout the application process. The transcript request form must be turned in to the Office of School Counseling at least 10 school days prior to your application deadline.

Letter of Recommendation Request Form - "green form"

The "Green Form" is used to request letters of recommendation from your teachers and/or counselor. Not all colleges require letters of recommendation. While some do not require them, they will accept them. Others will not accept them at all.

Mid-Year Report or Mid-Year Transcript Request Form - "purple form"

The "Purple Form" is used to request a Mid-Year Report or Mid-Year Transcript. Not all colleges require one, but if you need it, please submit a "purple form" to the Office of School Counseling.

Final Report or Final Transcript Request - No paper form

Final Reports/Transcripts will be sent based upon completion of the Naviance Graduation Survey taken in spring of senior year. There is no paper form to request a final transcript. You must indicate in the Naviance survey which college you plan to attend so your final transcript can be sent prior to summer break.

The "Yellow Form" is used to request a variety of documents, including transcript and letters of recommendation, that are often required when applying for scholarships.

The Athletic Transcript Release Form is used to grant the Office of School Counseling permission to release your transcript upon request by the college coaches and recruiters.
