WAIVER FORM

Each player that is signed up must have a parent/legal guardian acknowledge that they have read the following waiver information and click ACKNOWLEDGE on the registration form verifying that they have read this statement. By clicking ACKNOWLEDGE the player’s parent/guardian agrees not to hold the host schools; school representatives; coaches; officials; or other parties affiliated with this camp accountable for injuries or for damages/losses of personal property incurred by participation or attendance at any of the events that are part of the 2024 Future Stars Basketball Camp.  Acknowledging this on the form forebears any of the aforementioned entities/persons from responsibility for injury, illness, and/or damages/loss of personal property incurred by players, coaches, or family members while attending the camp.
