Immunization records must be submitted to and reviewed by the school nurse prior to school entry. Students entering grade 7 must have 1 dose of Tdap vaccine and 1 dose of Meningococcal vaccine prior to the first day of 7th grade. For your convenience, the MDPH’s immunization requirements are listed here. 

Exemptions: A physician’s certificate is necessary if immunization is contraindicated for health reasons. A parent or guardian’s statement is necessary if immunization conflicts with religious beliefs. Exemptions must be renewed on an annual basis.

A physical exam is required for all new students dated within 12 months prior to the entrance to school or 30 days after school entry and every 3-4 years after school entrance. Per these regulations, documentation of a current physical exam is required for all students entering grade 7. If the student is participating in athletics, an annual physical exam is required. Parents/guardians are encouraged to send a copy of the student’s physical to the nurse annually. The Massachusetts Physical Exam Form can be found here. Please have your child’s physician complete this form or provide a copy of their physical exam.
