Medications in School

If your child requires prescription medication at school a Parent Request for Giving Medication or Treatment form must be completed and signed along with a doctor's order and the medication in it's original labeled container. New forms and new MD orders are required for each school year. The Parent Permission for Over the Counter Medication form is sent home at the beginning of each school year and must be completed and signed in order for your child to receive over the counter medication at school. Please note that students are not permitted to transport medication in their backpack or on the school bus. All medications must be dropped off by an adult.
