I Do Not Have Documentation
and/or
I Need to Provide Additional Information
A student's self-report may provide evidence of disability
Accessibility Resources recognizes that students play a vital role in knowing how the disability impacts them and are a source in determining how a disability limits them in their lives and education.
Students who self-report having a disability can often identify how it currently impacts them in classes and on campus.
Historical information and the student’s self-report may be enough to describe how the condition impacts the student’s learning.
This is determined on an individual basis. No blanket statement will be made.
Option 1: Complete our Form-Fillable Student's Self-Report
Download the Form-Fillable Student's Self-Report
Directions to download, sign and return form-fillable documents electronically are available in writing and video.
Paper Self-Report forms can be found outside our office at RCH-105.
Option 2: Answer these Questions in a Written Self-Report
You may copy and paste the questions into a document and type or write in your answers.
