Accessibility Resources recognizes that students play a vital role in knowing how the disability impacts them and are a source in determining how a disability limits them in their lives and education.
Students who self-report having a disability can often identify how it currently impacts them in classes and on campus.
Historical information and the student’s self-report may be enough to describe how the condition impacts the student’s learning.
This is determined on an individual basis. No blanket statement will be made.

You may copy and paste the questions into a document and type or write in your answers.
What disabilities or barriers brought you to our office?
How long have you experienced your disabilities or barriers?
How are you currently being impacted by these disabilities or barriers?
Describe what classes you are typically impacted in. Is it all classes or just particular ones?
Describe when you are typically impacted. Is it just on tests or at other times as well?
What accommodations do you feel would be helpful and why?
What accommodations or strategies have you tried in the past (such as in high school, previous college, or at work), and how have they worked for you?
Describe any doctors or other diagnosing professionals you are currently working with, or have you worked with, specifically for the disabilities or barriers you are experiencing.
o If you have worked with any doctors or other diagnosing professionals within the last year, we would like further information from them regarding how you could be impacted at school and what their recommended accommodations are.
o You may use our Release of Documentation (ROD) form to request your documentation be sent directly to our office. Please fill out and sign the first page of the form, and return to our office. We will fax the form for you. You may also deliver the form directly to your provider.
Describe any medication you are currently on, including what medication and for how long.
Describe any medication you have tried in the past, and when and why you stopped.
Describe any other organizations you work with, or have worked with, due to your disability or functional limitations (i.e. Vocational Rehabilitation, County Health Department, etc.).
o If you have worked with other organizations, they may provide helpful information in planning accommodations. Our Release of Documentation (ROD) can be used to request this documentation as well.
Describe any relevant additional details.
Documentation or evidence of disability may be uploaded with your application, faxed, emailed, mailed, or brought into our office.
Accessibility Resources welcomes questions and recommends students contact our office directly if unsure about the documentation guidelines.
