Guide:

We require the Yellow highlighted sections to be pre-filled, starting from the ORI Code all the way down to the Level of Service
ORI Code, Authorized Applicant Type, & Type of License should already be filled out or provided by your employer
Agency Information should already be filled out or provided by your employer
Level of Service should either have one or two boxes filled out
The Blue highlighted parts are optional and should be provided by your employer. If it is for a license, always follow state instructions.
