All action plans last 1 year only and then will need to be updated by the provider.
When filling out the medication authorization form, if you write the provider's name, I will take care of getting their signature. If you would like your student to self-carry their emergency medication, please be sure to fill out the self-carry authorization form and write in the provider's name. Only emergency medications qualify as a self-carry medication in elementary schools. Thanks!
