School District Five uses an online registration platform called FinalForms, which is open all year long. At any point in the year, you may log into FinalForms to register a new student or update student information like addresses, phone numbers, etc. Please use the links below to help guide you through the process of registering students or updating their information.

4K Pre-K students for 24-25 will need to complete a paper application. Contact your student's zoned school for the application and more information.
5K Kindergarten students for 24-25 can access FinalForms. Please click "Register New K5 Student 24-25" above for more information. If your student is already enrolled in one of our Pre-K programs, then you will have access to FinalForms on May 1st.
All other new and returning students will be able to access FinalForms for the 24-25 school year on Wednesday, May 1st.

For new students:
Legal Birth Certificate with seal. A mother’s copy or a hospital copy is not acceptable.
SC Certification of Immunization. All immunizations must be recorded on a SC Certificate of Immunization.
Proof of Residency. Acceptable documents include a deed, certificate of occupancy, closing statement indicating completion or rental lease agreement with official signatures, utility bill (electric, gas, water, or telephone).
Custody Papers If applicable
Medicaid Card If applicable
For Middle and High School (Grades 6-12) A copy of your child's unofficial transcript (ideal) or latest report card from their previous school.
THREE (3) Proofs of Residency are required in order for your child to attend Lexington/Richland School District Five (LRSD5). Proof of residency will be done yearly for all students.
All documentation presented must show THE CURRENT MONTH’S DATE and The PROPERTY OWNER’S NAME OR the PARENT/GUARDIAN’S NAME.
Acceptable documents include a deed, certificate of occupancy, closing statement indicating completion or rental lease agreement with official signatures, utility bill (electric, gas, water, or telephone).

Contact your student's zoned school for any additional information.
Click on the "Update Student Information" button above the FAQ section.
Log into FinalForms and make any additions or corrections that you need to. (This can be done any time throughout the school year.)
Be sure to make the same changes for each student you have currently enrolled in District Five.
When logged into FinalForms, there are places where you have the option to upload documents such as proof of residence, birth certificates, etc.
If you are on a mobile device, you can simply take a picture of any documents you are trying to upload.
Be sure that the picture is clear and legible for others to review.
If you are on a computer, laptop, or device that does not have a camera, you will need to have your document scanned and saved to that device to upload it.
Reach out to your student's school for any questions or concerns you may have.

No worries. Go to our FinalForms site and click on Parent Login.
Use the "Forgot your email?" or "Forgot your password? options to get you back into your account.
There is also a "Get Help" button on the bottom of every FinalForms page.
If your student appears as "Withdrawn" when you log into FinalForms, simply click on "Click here to re-enroll" and follow directions to complete your student registration.
If your student has been enrolled in one of our schools at any point since the last day of the 2021-2022 school year, then they should already be in FinalForms.
Use the "Update Student" button above.
If your student has attended one of our schools in the past but was not enrolled on the last day of the 2021-2022 school year, then they should be considered a "New Student".
