Do you have an LPS issued device in need of repair?

1. Once you have completed the form, please wait 24 hours to ensure that we will have a replacement device for you. YOU WILL NOT GET A REPLY EMAIL

2. After the 24 hour period, you will go to your respective school main office with the broken device AND charging cable.

3. When you arrive, let the office know you are there to exchange a damaged device or a device that needs repair.

4. You will print your student's name on a sheet that will be provided to you and you will also list their broken device's asset number. The asset number is found on the underside of the device on a blue and silver tag or a white label. The office will keep the completed sheet that will then go with the broken device to our repair technicians.

5. You will be given a replacement device which will be the new device you will use. You will not get the damaged device back, but will continue to use the replacement device you are given.
