SNAP Parent Portal
The SNAP Health Portal is an online platform that gives you direct access to your student’s health record and streamlines communication directly with the William Diamond Middle School Nurses.
How to Register:
Parents/Guardians will receive an email from SNAP Parent Portal (from: noreply@studentehr.com) with login instructions including a username and password. Once registered, see instructions below.
SNAP Health Portal Log in:
Click www.studentehr.com to log into the SNAP Health Parent Portal.
If you forget your password, please click "Forgot Password?" under the password box to reset.
The portal has a language tab on the top bar of the homepage for you to select your preferred language.
IMPORTANT: Email notifications sent from the Diamond Nursing Department via the SNAP Health Portal will come from the email address noreply@studentehr.com.
You may navigate through the following sections which are found on the left side of the SNAP dashboard:
Demographics & Contacts:
This section includes your address, primary phone, insurance information, and contact information. Note that this information is pre-populated from the ASPEN student database. You will not be able to make demographic or contact information changes through the SNAP Parent Portal. Changes to your contact information should be submitted to the Diamond Main Office.
Please do NOT enter any information into the section labeled "Do Not Release To the Following People." This feature is NOT acknowledged by the nursing department. Any information updates related to contact restrictions need to go through the school principal.
Medications:
This tab will enable you to view all current medication orders on file for your student.
You may attach any prescribed medication order(s) from your student's health care provider and any medication parental/guardian consent form(s) which are required for the school nurse to administer medication to your student.
Medical Conditions:
Use this tab to review and update pertinent medical conditions diagnosed by your student's health care provider. Written documentation from your student's health care provider is required to confirm the medical condition. Please click "Attach Documentation" and upload the document as a pdf or jpeg. If the information is correct, simply press the CONFIRM button at the top of the screen.
Annual Physical Exams & Immunizations (Screenings Section):
Required for new enrollment, incoming 6th-grade students and student athletes
This tab allows you to upload your student's annual physical exam and immunization record. Click Attach Documentation and upload the record.
Letters:
This tab allows you to attach or upload health care provider's notes, physical exams, or any test results that you would like to have in your student's health record.
This tab also allows your school nurse to attach forms which you may then (electronically) sign and send back to the school nurse.
Messages:
This allows for you to communicate all non-urgent information with the school nurse during the school year. For matters requiring immediate attention, please call the Diamond Health Office at 781-861-2460 x 2.
Forms:
This page shows all forms and handouts used by the school health office. You may be asked by the health office to read, sign or fill out these forms.
