a. Channels for Reporting: Individuals are urged to report any concerns through the appropriate channels such as a supervisor, manager, designated ethics officer, or an anonymous hotline.
b. Content of Report: Reports should contain as much detail as possible, including the nature of the wrongdoing, names of those involved, dates, locations, and any supporting evidence.
c. Anonymous Reporting: Individuals may choose to remain anonymous when reporting concerns. However, providing contact information may assist in the investigation.
