Guidance Notes

New Student Requirements:

1. Copy of birth certificate

2. Copy of Social Security card

3. Up-to-date immunization record (including the Tdap booster before student can enter 7th grade)

4. Current physical address and telephone number(s)

5.  Parent/guardian information (custody papers if applicable)

6. 504/IEP plans if applicable

7. Transfer school information to include attendance, discipline, transcript, and testing

If you'd like to have a conference with your child's teacher(s), please call me at 346-1011 so we can setup a meeting time during the teachers' planning block or before/after school.
