First Aid/CPR

Please print the form below, and return it to the cash cage with your $100 fee.  Upon completion you will be fully certified in First Aid CPR C with Red Cross.  In addition you will receive HFR units (3) and two Sports Medicine credits (HCS 2020 and HCS 3040).

Details will be sent to the email that you provide as to how to access the online theory portion.   This online portion must be completed by April 30.  Please note that refunds will not be issued once you have been registered in the online portion.
