To comply with state law, please complete the 2025–26 Health Services Annual Parent/Guardian Consent Form to indicate your decision.
This ensures we provide only the services you authorize.

Dear Wiley Parents/Guardians,
Welcome! I am so excited to be your campus nurse and would like to take this opportunity to share some information with you on ways we can partner to make this a successful school year.
Medications: For the safety of our students, all medications are kept in the clinic. All clinic forms can be found on the Leander ISD Health Services webpage, including medication procedures. We encourage you to review all medication procedures and contact me if you have any questions.
Illness/Hospitalization: If your child is sent home with fever, vomiting, diarrhea or other contagious illness, they must be symptom free for 24 hours without the use of medication before returning to school. If your child has been prescribed antibiotics, they must receive the medication for 24 hours prior to returning to school.
If your child has been hospitalized, a note from the student’s physician stating the date the student is allowed to return to school is required. Students who come to school with an injury requiring interventions such as surgery, casts, crutches, wheel chairs, etc., must have a physician’s note prescribing what activities the student is/is not allowed to do while under the physician’s care for the injury.
Immunizations: If you receive a letter, please try to get your child’s updated immunization record to the clinic as soon as possible. If the updated record is not received on or before the recommended date on the letter, your child will be excluded from school.
State Mandated Screenings: The following state mandated screenings will be performed according to the Texas DSHS guidelines:
Hearing and Vision
7th Grade
All students new to Texas schools
Acanthosis Nigricans (physical skin finding that may be a marker for high blood insulin levels)
7th Grade
Spinal
Females in 7th Grade
Males in 8th Grade.

Student Health Information Changes: If your child’s medical condition changes at any time, please submit a Student Health Information Update.
My goal is to keep your child healthy, safe and in class so they can learn and have a wonderful elementary experience.

Administration of OVER-THE-COUNTER medications during the school day REQUIRE a medical provider AND parent/guardian signature.
This includes but is not limited to: Ibuprofen or acetaminophen, oral or topical allergy medication, insect bite or burn-gel.
