All student athletes looking to participate in a UIL sanctioned sport at Vista Ridge High School will need to complete the following prior to participation. 

1. Pre-participation Physical Evaluation Form

  • UIL Sudden Cardiac Awareness (SCA) Form

  • UIL Acknowledgment of Rules 

  • UIL Safety Training 

• Emergency Contact Card

  • Medication Consent & Eligibility 

  • ECG Testing Opt-In 

  • Texas Children's Urgent Care & Telemedicine 

3. Sway Balance Baseline Test

• Download the Sway Medical app on your mobile device

• Contact the athletic trainers and / or your head coach for a testing code

NOTE: Students are required to have a physical completed annually along with the previously mentioned forms.
