The team Strike Out Form will be used by the entire 7th Grade as Discipline/ Reward tool. Each student will be given one warning per category before a lunch dhall is assigned. After three lunch dhalls the student will be given an office referral. At the end of each six weeks the students will get a new card and start over. Also, any students who have no signature on their card will be enterted into a raffle for various prizes.

*If a student loses his/ her card ALL warnings will be eliminated along with four passes being taken away.
