All LHS students that are attending prom MUST complete a LHS Prom Contract. The Prom contract will be done via Google Form this year.  The form MUST be completed by the LHS Junior / Senior purchasing the ticket.  As well if you are attending as a Junior or Senior guest that is currently attending LHS. The Prom Contract is attached below and MUST be completed prior to prom tickets being distributed to the student.  

*Students who have completed the signed contract (Google Form)  and do not owe hours will be issued a ticket when they are distributed. 

There are NO TICKET REFUNDS *. 

Hours are to be completed by April 10th by 3:35pm.

The Prom Guest Form must be completed for any non-LHS Student, a LHS Freshman or Sophomore that is attending prom with a current LHS Junior / Senior. This is printable form / paper; hard copies (please do not ask for share permisson - please download the form). A few hard copies will also be available in the Back office with Ms. Barrera. This Form  MUST BE TURNED IN INTO THE BACK OFFICE, to Ms. Barrera.)  

You MUST provide A printed copy of the guests student ID or Drivers License and it must be submitted attached with the form. DO NOT EMAIL.

LHS Administration will let you know if your guest is approved or not.
