2020-21 Household Application for Free and Reduced Price School Meals: This is a required form for all families that must be received either by printing out or attached to an email. If you are unable to get the PDF version of this form printed or emailed, please contact the school office at 715-588-3838.

Directory Information Non-Release Form: Opt out of release of student information (name, photo, grade, awards, participation in activities/sports, parent name) that may be published to school (i.e. newsletter, website/social media, yearbook) or local media releases for the purposes of recognition.
Student/Family Residence Questionnaire: Determine eligibility for additional services for homeless students/families.
You do not need email or a printer to complete these forms. The information will be sent directly to the school upon submitting. These forms can be completed on any type of Internet-connected device (phone, tablet, or computer)
