Quick reminders for the 2023-24 year:
Students,
You must fill out a nurse referral form in order to visit the nurse office for any reason other than a medical emergency during class time.
- Medical emergencies include, but are not limited to: high or low blood sugar, active vomiting or bleeding, asthma attack, allergic reaction, seizure.
- The Google form should be in each of your Google classrooms. I also emailed the link to all students.
- You may still come by my office before school, between classes or at lunch without filling out this form.
- When you fill out the form, I will be notified of a new request. Do not report to the nurse office until I call for you. Please let me know if you have any questions. See the link below.

Medications: In order for you to have or take any medications at school, you must have the appropriate forms on file with the nurse office. If you have any questions, please see me.
Extra Bathroom Privileges: If you have a note on file for extra bathroom privileges from last school year, you will have to submit a new note from your physician to continue these privileges. Please see me if you have any questions.

Dental Van Access
The dental van is still coming to schools for students. They have the same cleaning/safety guidelines in place as a normal dentist office would. If you would like your student to be seen by the dental van staff for dental hygiene, please click on the link below to fill out the form. New Guidelines for School Administration of Medications
August 2022
Dear Parent,
We are strongly committed to the health, safety and wellbeing of our students. This is best achieved when we work in partnership with you, physicians and where possible, your children. Our school has Guidelines for School Administration of Medication to ensure the safe administration of medication to students during the school day. Any medication, prescription or over-the-counter, must be delivered to the school nurse by the parent or legal guardian. Please do not send medication with the student. Prescription medication must be in the original labeled container provided by the pharmacist. Over-the-counter medication must be in the original, unopened container with manufacturer’s label. Medication to be given at school must be accompanied by the Permission for School Administration of Medication form. A separate form must be completed for each medication. Beginning in 2019, a doctor’s signature was required for prescription medications.
Parents are responsible for informing the school of their child’s potential for anaphylaxis and providing ongoing health information and necessary medical supplies. If your child has been prescribed emergency medication, such as an EpiPen or a rescue inhaler, please notify the school and provide medication to the nurse to have on site. A student may not carry medication on their person without physician permission, parent permission and school district permission to self-administer and self-monitor. If you would like to request your child carry their own EpiPen or rescue inhaler, please contact your school nurse.

I have a Bachelor's Degree in Nursing from the University of South Carolina Upstate. I have been a practicing nurse for the past 23 years and this is my 10th year as a school nurse in Lancaster County. I hail from Spartanburg, SC, but have lived in Kershaw for the past 21 years with my husband and our kids. While I did not graduate from AJHS, I still yell GO VOLS!!
