We go on several field trips during the school year and I will need parent chaperones for each trip. You need to be cleared before you can chaperone. Please fill out a Volunteer Driver Application Form and read the instructions. In addition to filling out the form, you must attach a copy of your driver's license and current vehicle insurance card. Once that has been completed and you have the copies, please email it to me or Bonnie Strock (bstrock@lcs.org) or drop it off in the office. You will then get an email with a link for a background check so you will need to complete that as well.
If you have already been cleared, you don't need to fill out a new form every year, but make sure the office has a current copy of your insurance card (which, as you know, expires every 6 months) and your driver's license.
Please do this as soon as possible because it takes some time for it to be processed and this way you'll be all set to drive when needed. Thank you very much for your help!
