Medication at School Forms
Any medications to be kept and taken at school by students must have a doctor's note/order.
If you anticipate that your child will need medication during school hours, please print the Medication at School Form (English & Spanish versions avialable) and have your child's doctor fill out the form to return to the school nurse. Medications provided to the school's health office must be in the original packaging with the child's name along with the medication label and instructions. Medications will be kept in the health office and will need to be picked up at the end of each school year.
If your child has multiple medications each one will need a Medication at School Form.
For students in 5th grade or higher, parents have the option of having their doctor fill out the Permission to Carry form. This form will need to be returned to the school nurse before the student is allowed to carry the medication to school.
Every new school year your child will need to get new orders from your medical doctor using the Medication Forms.
