Any medications to be kept and taken at school must have a physician's order. If you anticipate that your child will need medication during school hours, please print the Medication at School Form here or using the button below, (English and Spanish available) and have your child's doctor fill out the form to return it to the school nurse. Medications provided to the school's health office must be in the original packaging with the child's name along with the medication label and instructions. Medications will be kept in the health office and will need to be picked up at the end of each school year. If your child has multiple medications each one will need a Medication at School Form. . Each school year, the school requires a new Medication at School Form for each medication to be taken at school.
Asthma, allergies, diabetes, and seizures are all common health conditions we routinely care for in the school setting. If any of these health conditions apply to your student, please click the buttons below to access any of the school required forms for Asthma, Allergies, Diabetes, and Seizures. Just like medications, many of these forms need completed by a physician as well as signed by a parent.
Please call the school office for enrollment questions and additional requirements. Health related requirements for enrollment include current immunization records providing proof of state required vaccinations, a completed permanent health history, a completed Oral Health Assessement by the end of first grade, and a Report of Health Examination for School Entry. Students with food allergies or specific meal requests for medical reasons should have a Meal Accommodation Form completed and submitted to nutrition services. Please find the form below. Please contact the School Nurse or Nutrition Services with any questions regarding meal accommodations at school.
At the end of the Spring Semester all 5th Grade Students will be asked to complete a Health History Update Form before going to middle school. Or, if your student of any grade has had any major health changes since completing their Permanent Health History Form on enrollment to LBUSD, please complete the Health History Update Form found here and return the completed form to the school nurse.
For initial IEP evaluations or IEP Reevaluations please complete the form found here and return the completed form to the school nurse. Your child will come home with a packet of the required paperwork as well.
