The district contracts with Ochsner Health Systems – Workforce Wellness to conduct all drug and alcohol screenings. We have a form you must take with you, do not go on your own.

Registration Requirement: All drivers are required to register with the Drug and Alcohol Clearinghouse. If you do not have an account, you must create one at https://clearinghouse.fmcsa.dot.gov. Upon registration, you will receive an email from clearinghouse@dot.gov to provide consent electronically.
Consent Requirements:
For a Full Query: You must sign an electronic consent form allowing FMCSA to release detailed information about any drug and alcohol program violations in your Clearinghouse record. This consent is required annually.
For a Limited Query: LCSD will provide a consent form for you to sign, specifying the duration of consent for periodic limited queries. This consent is effective for the specified period and does not require annual renewal.
Consequences of Non-Compliance: Failure to provide consent will result in your inability to operate a commercial motor vehicle or perform other safety-sensitive functions for Lauderdale County School District, per 49 C.F.R. 382.703(c).

Lauderdale County School District will conduct a limited query of the FMCSA Commercial Driver’s License Drug and Alcohol Clearinghouse to determine whether drug or alcohol violation information about me exists in the Clearinghouse. Your consent is for multiple limited queries through my employment with Lauderdale County School District.
Limited queries conducted by Lauderdale County School District indicates that drug or alcohol violation information that exists in the Clearinghouse, FMCSA will not disclose that information to Lauderdale County School District without first obtaining additional specific consent from you.
Refusing to provide consent for Lauderdale County School District to conduct a limited query of the Clearinghouse, Lauderdale County School District must prohibit me from performing safety-sensitive functions, including driving a commercial motor vehicle, as required by FMCSA’s drug and alcohol program regulations.
