These are all the forms you will need to attend the prom.

Permission Slip

Sr Prom Permission Form 2021

You will need to print this, you and your parents sign this, and you turn it in the activities office. All LC students attending the senior prom, including non-seniors, need to fill out this form. Due April 21st!

Senior Prom PowerPoint and Ticket Sales

This is a Google Form with a PowerPoint attached all LC students attending the prom should watch and fill out, including non-seniors and students who are not paying for their own tickets. Open April 17th - 21st.

Outside Guest Form

PERMISSION SHEET FOR OUT-OF-SCHOOL GUESTS

This is a form that needs to be filled out in order to bring a non-LC student to the prom. Due April 21st!

Dietary Restrictions

This is a Google Form to fill out if you have any dietary restrictions you need addressed. We will do our best to accommodate all restrictions. Due April 26th!
