These are all the forms you will need to attend the prom.

Permission Slip

Jr Prom Permission Form 2023

You will need to print this, you and your parents sign this, and you turn it in the activities office. All LC students attending the junior prom, including non-juniors, need to fill out this form. Due March 26th!

Junior Prom PowerPoint and Ticket Sales

This is a Google Form with a PowerPoint attached all LC students attending the prom should watch and fill out, including non-juniors and students who are not paying for their own tickets. Open March 18-March 26th.

Outside Guest Form

PERMISSION SHEET FOR OUT-OF-SCHOOL GUESTS

This is a form that needs to be filled out in order to bring a non-LC student to the prom. Due March 26th!

Dietary Restrictions

This is a Google Form to fill out if you have any dietary restrictions you need addressed. We will do our best to accommodate all restrictions. Due April 5th!
