Official Transcript Request Form

How do I get my transcript sent to colleges, employers or the military of my choice?

Download the transcript request form
Fill it out: 1) indicate when we should send your official transcript; 2) list the name and address where the transcript is to be sent
Fax the request to 660/347-5437
Email the request to aanderson@lmvikings.org or cpummill@lmvikings.org
Students (current or former) may drop the request by the main office

Can my transcript be faxed to a college or employer?

Answer: Yes, please provide the fax number for the employer or institution

Can my transcript be emailed?

Answer: Yes, please provide the email address for the employer or institution

Can I get a copy for myself?

Answer: Yes, a personal copy of your transcript will say "issued to student"

Can I pick up an official transcript and take it to my college or employer?

Answer: No, official transcripts must be sent directly from the high school
