Network Nurse Program

The Network Nurse provides outreach to Antioch area residents through home, office, and phone visits. Her solution based focus is through individual health counseling and referrals to AAHAA’s healthcare service partners. Health outcomes are improved by increasing access to healthcare and minimizing barriers such as lack of awareness, financial hardship, and lack of transportation. The nurse is available for one on one meetings to help residents more fully understand healthcare issues such as medical conditions, medications, and treatments. This program also provides educational seminars based upon a community needs assessment and outreach activities.

Transportation Program

AAHAA provides curb-to-curb transportation to and from medical appointments for Antioch area residents. Availability is Monday thru Friday, on a limited basis. Service is free, but donations are always welcome. Call now to reserve a seat, on a first come first served basis.
