The Village of Lakemoor proudly announces an exciting new partnership between the Village and Antioch Area Healthcare Accessibility Alliance for a trial period.

Antioch Area Healthcare Accessibility Alliance is a nonprofit organization dedicated to making sure that all our neighbors are connected with and receiving the healthcare services they need and can afford.

If you or a loved one, friend ,or neighbor are having trouble finding your way through the healthcare maze, don't even know where to begin to find help, or just want a little guidance about what to do, contact us directly at 847-395-2809 and we will be there to help you figure it out.

Our Network Nurse can schedule an appointment to visit with you one on one to address any of your healthcare needs or concerns . They can direct you to and connect you with resources that will best meet your individual needs.

You may also contact us if you or someone you know needs a ride to and/or from a medical appointment. Whether it be physical, dental, vision, mental health, dialysis, physical therapy, etc. we are here for you and want to help and will do the best we can to get you to your appointments.

Antioch Area Healthcare Accessibility Alliance, AAHAA - Connecting you to healthcare resources because we care.

The Network Nurse provides outreach to Antioch area residents through home, office, and phone visits. The Nurse's solution based focus is through individual health counseling and referrals to AAHAA’s healthcare service providers. Health outcomes are improved by increasing access to healthcare and minimizing barriers such as lack of awareness, financial hardship, and lack of transportation.

The nurse is available for one on one meetings to help residents more fully understand healthcare issues such as medical conditions, medications, and treatments. This program also provides educational seminars based upon a community needs assessment and outreach activities.

AAHAA provides curb-to-curb transportation to and from medical appointments for Antioch area residents. Availability is Monday thru Friday, on a limited basis. Service is free, but donations are always welcome. Call now to reserve a seat, on a first come first served basis.
